Sample Submission Forms

When send samples to us please use the appropriate form below to accompany your samples.

We’ve implemented a new, easier to use and specific Sample Submission Form which is now to be used for any sample submitted for casual testing. 

What’s new:

We have split the submission form into 3 forms - specifically designed around the species type, pick from;

• Samples must be accompanied with a form, we will no longer accept samples without a form, or reference to an existing "saved form"
• MTNZ Sample submission form to be used, please not alter your own to adapt to this form, this is part of our quality control document and accreditation process
• All forms must have a reference number that can clearly be identified on the physical samples if the form is not sent with the samples and emailed instead

Action Required:

• Start using the new appropriate MTNZ submission form.
• Ensure all samples submitted have a submission form.
• Ensure all samples have reference number easily identified on the form.

Transition Period:

• For the next three months, the old submission form will still be accepted, however;
• After this period,  from 1 March 2026 old forms will no longer be acceptable, and samples will be held in query until the new form is supplied, up to 48 hours from which samples will then be destroyed

These changes are designed to formalize our process and make it more efficient for both you as the customer and MTNZ.
